• Wik L, et al. Delaying Defibrillation to Give Basic Cardiopulmonary Resuscitation to Patients with Out-of-Hospital Ventricular Fibrillation: A Randomized Trial. JAMA Mar 19 2003; 289 (11): 1389 – 1395.  　　　VFアレスト。目撃ありならまずショック。目撃なければまず胸部圧迫ですね。

• Abella B, et al. Quality of Cardiopulmonary Resuscitation During In-Hospital Cardiac Arrest. JAMA Jan 19th 2005; 293(3): 305 – 310.　　　　まだまだハイクオリティ胸部圧迫できていない。全体の24%の時間で揉んでいなかったとのこと。個人的には真面目な日本人のほうがCPRの質が高いと思います。現状把握の素晴らしいスタディ。

• Hallstrom A, et al. Cardiopulmonary Resuscitation by Chest Compression Alone or With Mouth-to-Mouth Ventilation. NEJM May 25th 2000; 342 (21): 1546 – 1553. 　　　CPRの都、シアトルから。素人のCPRでは、胸部圧迫のみ群とMouth-to-Mouth付き群でサバイバルに有意差がなかった。Cardiocerebral Resuscitationの先駆け。

• Kellum MJ, et al. Cardiocerebral Resuscitation Improves Neurologically Intact Survival Patients with Out-Of-Hospital Cardiac Arrest. Ann Emerg Med 2008 Sep; 52 (3):253. 　　　新しい時代の幕開けか？病院外、ショック可能なリズムで挿管／バッギングなしで酸素マスクのみを10分間。ひたすら揉み続けたら生存率も神経予後も改善。CCR (Cardio-Cerebral Resusciation)の時代の予感。

• Garza AG, et al. Improved Patient Survival Using a Modified Resuscitation Protocol For Out-of-Hospital Cardiac Arrest. Circulation May 2009; 119 (19): 2542.　　　　これまたCCRをサポートする結果。 後ろ向きに従来のACLS群と、挿管なし／gentle ventilation (胸部圧迫：換気 = 50:2)で揉みまくる群を比較。後者の生存率、神経予後が有意に高かった。

• Hypothermia After Cardiac Arrest Study Group. Mild Therapeutic Hypothermia to Improve the Neurologic Outcome after Cardiac Arrest. NEJM Feb 21st 2002; 346 (8): 549 – 556.　　　　VFアレスト。冷やすと死亡率、神経予後ともによし。有名な論文ですね。

• Bernard S, et al. Treatment of Comatose Survivors of Out-of-Hospital Cardiac Arrest with Induced Hypothermia. NEJM Feb 21st 2002; 346 (8): 557 – 563.

• Olasveengen TM et al. Intravenous drug administration during out-of-hospital cardiac arrest: a randomized trial. JAMA. 2009 Nov 25;302(20):2222-9.　　　　　ついにでましたね。ACLSにおける薬剤投与にとどめを刺しそうなスタディ。ノルウェイで非外傷性の院外心停止患者を、薬剤投与付きACLSと薬剤なし群にランダム化したRCT。入院時の心拍再開生存率は薬剤投与群で有意に高かったが、退院時生存率と一年後の神経学的アウトカムに有意差なし。ACLSガイドラインに影響を及ぼしますな。
